Case Study 1: Mr. K’s

The Registered Nurse gives a brief medical history of the patient that is significant during assessments and diagnosis processes. Since assessment of patients is key in nursing, nurses should carry out timely assessments on the overall physical, emotional and medical history of the patients as already portrayed in the case study (Baid, 2006: Munroe, Curtis, Considine, & Buckley, 2012).

Nurses should use critical thinking in directing further medication and offering care to patients. Because the patient was in great pain, he was prescribed to take paracetamol to relieve it. It is evident that the nurse understands the importance of clinical reasoning cycle and that’s why the nurse proceeded in conducting more assessments to gather relevant clues that guided subsequent medication (Banning, 2007). Further assessment directed to a possibility of either a renal calculi and UTI or a Urinary tract infection which necessitated for an x-ray to ascertain the cause of pain. The X-ray indicated presence of renal stones that were successfully removed. The nurse portrays a good mastery in the assessment, use of clinical reasoning and critical thinking skills in the care of patients as evident in the case study (Jarvis, 2011).

Case 2: Hepatic encephalopathy case study

The Registered Nurse has presented his case study very well by clearly articulating overall physical, psychological and medical history of the patient as required in the nursing profession. The nurse noted the need for insertion of Nasal-Gastric after assessment, but since the nurse understands the complication associated with the undertaking and is not within the nurse’s line of duty, the nurse had to ask for assistance. This illustrates a good application of critical thinking skills in offering care for the patients. Nurses should make right judgments in care delivery (ANA, 2010).

The nurse portrays a good use of the clinical reasoning cycle framework by administering subsequent assessments to the patient to direct ensuing medications (Banning, 2007). In addition, by assessing the patient’s vital organs every four hours, the nurse showed mastery in the principal of ‘between the flags’ where patients who are deteriorating are assessed on possible damages of vital organs every 4 hours in order to administer timely interventions. The nurse’s consultations show that the Registered Nurse embraces the concept of teamwork and shared responsibility in offering care to patients. The nurse recognizes the importance for baseline assessment, clinical reasoning for timely diagnosis and intervention (Ferguson, 1998).

Case Study 3: Medical Ward

The nurse understands the importance of timely assessment and diagnosis in enhancing quality of care for patients. The nurse precisely highlighted the baseline assessment of the patient and immediate medication offered. During the time span that the nurse was observing the patient, the nurse demonstrated use of clinical reasoning by conducting continuous assessment to gather important clues to direct further treatment (ANA, 2010). In addition, the nurse appreciates teamwork as the case study showed constant consultations while offering care to the patient (Banning, 2007).

The writer has brought about an important issue of misdiagnosis in patient medication. Since the patient had undergone amputation of the leg, the nurse focused only on characteristics of a well healing wound (Brown & Edwards, 2012). The nurse accepted a failure in conducting thorough assessments on the patient. This failure resulted to the deterioration of the patient’s health until he was referred to higher level facility. Nonetheless, the case study was very educative as the nurse emphasised on the importance of the overall assessment of patients both cognitively and physically as well as proper use of clinical reasoning framework in identifying relevant clues to review prescribed medication.
