Preparation for the transition to graduate practice, clinical practicum

A registered nurse needs to maintain competency standards in order to provide the necessary services to the patient. These standards provide an assessment framework on the competency of the nurse in order to improve their professional practice according to society’s health needs and the observed change in patterns of illness (Australia nursing and midwifery council, 2005). The aim of this assignment is to assist students to develop strategies that will assist them in achieving the ANMC Competency standard for the Registered Nurse during the TGP clinical practicum.

Professional Practice

This involves professional, ethics and legal responsibilities that applies demonstration of knowledge base which is satisfactory, accountability in practicing and working according to the laws affecting nursing and health care and also protecting the rights of groups and individuals(Australia nursing and midwifery council, 2005).

My learning objectives in this domain are to practice in accordance to ethical, legal and the professional nursing framework and performing the interventions in nursing to the standards that are recognized for practice.

The standards will provide me with guidance to the skills attitudes, knowledge, and judgments that are required for safe practice. These standards also outline what the profession of nursing expects from me, provide guidance, direct the nursing practice as a profession, provides a framework for the development of competencies and also assists me in respecting and understanding of different roles in nursing. (Creasia & Peaker, 1996).

Standards of a professional performance

Quality of practice will enhance the effectiveness and the quality of my practice as a nurse. The criteria of measurement includes; documentation of the application of the process of nursing in a responsible , ethical and accountable way, initiating changes in nursing practice using activities of quality improvement, applying creativity to improve nursing care delivery and participation in quality improvement of activities.

Education will assist me as a nurse to attain competency and knowledge that reflects the nursing practice in current situation. The measurement criteria involves participation in education on clinical and knowledge professional issues, taking action in achievement of identified goals in evaluation process and providing a care that is age appropriate in a manner that is ethnically and culturally sensitive.

Professional practice evaluation will help me to evaluate my own practice in nursing relative to the standards of professional practice, rules and regulations and statutes that are relevant. Collegiality standard will enhance me to interact with peers and other health care providers in order to contribute to professional development. I also need to collaborate with all parties involved in conduction of nursing practice such as patient , members of the family and others. Ethics will lead to integration of the provisions on ethical issues in the areas of practice. Through research, I will be required to integrate all the research findings during the practice.

By utilization resources, I will need to put into consideration the factors that are related to cost and safety effectiveness during the delivery and planning of nursing services. Finally, I am required to provide leadership in the setting of professional practice and also the profession (Nursing Management, 2010).

Critical thinking and analysis

Critical thinking is said to be a core competence that has been identified to be essential for learners to become self-directed. The critical thinking process involves both deductive and inductive thinking. As a critical thinker, I will have to relate concepts and determine relationships that are casual. The nurturing of critical thinking is through a combination of the academic and the clinical settings of nurses (Jacklyn et al. 2004).

My learning objective in this domain is to practice on the basis of a framework that is evidenced based.

Evidence based practice make up a process of building questions that includes clinical findings, therapy, diagnosis, and prevention of diseases. The process of question building will bring up the question that is often encountered in practice, the relevance of this question to the situation of the patient and in practice. Evidence based practice will be better understood as a framework for decision making which will facilitate decisions that are complex in different and conflicting groups. It involves considering research and other forms of evidence on a routine basis when making health care decisions.

Health care decisions that am required to make include management of test for individual patients, choice of a treatment and also decisions on policy for large populations (American Society of Registered Nurses, 2007).

Critical thinking criteria involves

Assessing the patient.

Evaluation will assist me to determine how well the outcomes will be achieved, if there is any help required and how better this can be done.Diagnosis will involve a dynamic process of identifying the risk factors, diagnosing the problem and predicting the potential problems. Implementing will consists of determining the appropriateness of the interventions, putting the plan into action, to reassess and modification of a plan when necessary.

Provision and coordination of care

The domain involves a process of coordinating, organizing, and providing a nursing care that consists of groups or individual assessment, to plan, implement and to evaluate care (Australia nursing and midwifery council, 2005).

My learning objectives involved in this domain include using reasoning cycle in a clinical way as an approach to client care which is structured and using a range of techniques such as observation, physical assessment, interview and mental status examination and client’s history collection in order to conduct a comprehensive assessment in nursing.

The strategies that I will use to coordinate care can be divided into two groups. First are the processes that clinicians use in order to coordinate care such as communicating with service providers, supporting service providers and the patients. For instance, a communication in conferences with an expectation a specialized team of members will provide information on patient progress and the changes in care. Second are the strategies that relate to structural arrangements that are derived with an aim of supporting the coordinating activities. This include systems that are used to coordinate clinical activities in order to promote a continued care such as coordinated consultations, shared assessments and making arrangements to provide accelerated access to services for patients (Gawaine et al. 2006).

Collaborative and therapeutic practice

This domain involves establishment, sustainment, and conclusion of relationships that are professional with groups or individuals. It also consists of competences that relate to the understanding of the nurses and their added support to the team of interdisciplinary health care (Australia nursing and midwifery council, 2005).

My learning objectives to this domain are to establishing a relationship that is therapeutic and at the same time maintaining professionalism boundaries, ensuring there is safety in the way medicines are administered by using a quality medicine framework, also establishing a working relationship that is collaborative and positive. Adhering to legislations on occupational health and safety, effective use of verbal communication skills and using the principles of ergonomics with an aim of avoiding injuries and to improve the comfort of the client.

As a nurse I need to adopt strategies that are effective in order to be successful in managing the boundaries related to therapeutic relationships. These strategies include;

  • A better understanding of the limits in therapeutic relationships,

  • Involving the client in establishing, communicating, and also maintaining boundaries which are professional to the client.

  • Implementing a practice that is reflective.

  • Keeping off therapeutic and social relationship as much as I can.

  • Bringing to an end or trying to avoid therapeutic relationship.

  • Making communications on the expectations and the limits for confidentiality.

  • Being sensitive to the manner in which care is being provided (CRNM 2011).


This assignment will enable novice nurses to gain skills and demonstrate competence in providing health care. Through this, I will be able to provide a nursing care that is evidence-based to all the people irrespective of cultural groups, age in consideration of individuals, communities, and families. I will also be able to maintain and promote health and prevent illness for people with mental illness, rehabilitation and disability needs, and reduction of pain during the end stages of one’s life.
