Assistants in Nursing
Assistants in nursing are considered a valuable asset to any health care organization. They help with the administrative work mostly relieving the nurses from such duties so that they can fully concentrate on patient care (Chiarella, 2009). The AIN’s also benefit from this as it acts as a stepping stone to the development of their nursing careers by creating a path as they learn along the way. However, a structure exists from the top where the AIN’s are at the bottom. The AINs however face quite a number of problems in the healthcare industry.
Unwarranted pressure- in a bid to cut down on costs, more AIN’s are being employed in favor of RNs. This is because a fully qualified and registered nurse requires more pay hence health care centers tend to have fewer nurses and more AINs. This puts undue pressure on the AINs as they are only supposed to be nursing assistants (Carrigan, 2009 p.25). If all AINs depend on one RN, the work load becomes more since they are not qualified and the provision of services may be questionable and the organization can be said to be socially irresponsible. It is also a fact that while some AINs may want to work hard so as to advance in their careers, some have no idea on what they are doing and may pose a risk to the patients. The ratio of patients to nurse must apply with the AINs as a supplement and not a substitute. The AINs are only valuable when the necessary ratios apply.

Horizontal violence is a type of nonphysical hostility that portrays criticism, undermining and some form of sabotaging towards someone (Duffy, 1995). Members in the nursing profession are often faced with a struggle on a regular basis. This kind of oppression that occurs in medical institutions may make it difficult for the nurses to overcome the negative emotions portrayed towards them and lose all power to control their feelings hence become aggressive to their counterparts. According to (Yonge et al 2002), student nurses were more prone to horizontal violence in the field and had higher levels of stress when preparing for their academic examinations. This was due to the fact that these students had to interact with their preceptors during the whole exercise as they would help them in the preparation (King-Jones, 2011). The psychological impact that is caused with the certain acts of horizontal violence can have negative effects on the students and affect their performance levels and they may end up despising the nursing profession (McKenna et al 2003). The relationships, if unsuccessful, would leave the students frustrated or disheartened which would lead to stress and disillusionments with regards to the profession. If Foucault’s method of analysis; power/knowledge, one can gain insight on gaining information on the nursing practices and devise ways on how to overcome the oppressive practices. Nurses can be encouraged to be more open within the health care system and be more transparent as hidden agendas lead to passive aggression tendencies.

Retention of RNs in Nursing
In the health care industry, nursing has the highest growth rate. At the same time, the retention rates of nurses especially the young ones is quite poor. This is due to the pressure mounted on the RNs who are physically exhausted from the increased patient overload that leaves them emotionally drained. The conditions under which they work are also non-conducive for them. For the retention rates to be higher, the nurses need to be satisfied with their jobs. Job stress is one of the most common factors that affect the job satisfaction level of nurses (Read, 2013). The many cases of turnover can be accredited to the working environment, sacrifice or shock (Dotson et al 2014). The treatment that the nurses receive is not at par with their other colleagues in the same health care industry. Their level of knowledge is not acknowledged by other physicians and they are not accorded the respect that they deserve. This can lead to demoralization and the nursing profession can be seen to be unworthy. In addition, time management in nursing has become an issue that prevents proper provision of their services. The patient to nurse ratio should be suitable to allow the nurses to perform their duties efficiently without being fatigued or overwhelmed. If the nurses have no time for their patients, personal interactions cannot be developed leading to further increases in job dissatisfaction.
