The placement rationale is to help me understand the contemporary issues surrounding the nursing practice such as the culture and ethos. It will also help me understand the complexities and challenges that are faced by the nurses. Additionally, I will impact significantly on the community through the reduction of mortality rates and critical incidences such as medication errors, insufficient patient care resulting in physical fall, pressure ulcers as well as the infections that are acquired in hospitals (Levett-Jones & Bourgeois, 2011).

The placement practice will provide me with an opportunity to apply what I have theoretically learned in about wound dressing in a practical sense. The wound dressing techniques begin with simply identifying the existing causes of pain and the potential causes of wound at rest. These range from wound aetiology to infection and arthritis. Since the pain may be caused by other non-wound related pathologies, the placement will help in me in making a more practical sense to it (Crisp et al., 2013).

Other wound related issues include location of wound pain through body map by gentle touch. This is followed by the pain reduction methods such as analgesia, placing patients in a warm environment and the elevation of the feet. These procedures are more individual related and therefore I will be able to explore more about pain reduction techniques. Another important technique is in regard to the methods used to secure a dressed wound such as use of adhesive tape and retention bandage. Since these can cause a lot of pain during the removal process due to hypersensitivity in the area surrounding the wound, care should be taken not to cause formation of oedema which may result into traumatic pain.

I will also learn about cleansing agent and techniques, identifying infected wounds, other strategies that can be used to release pain such as the self-removal of the dressing, taking breaks or time-outs and distraction techniques.

The administration of oral medication in the placement will provide that challenging environment where high level of concentration is paramount. The Oral medication procedure is one area where there is no room for error. The clinical placement will help in enhancing my precision and accurateness in administering the oral medication procedures. Simple procedures like washing hands must never be forgotten and this placement setting will help in ensuring that the procedures are at my fingertips (Levett-Jones & Bourgeois, 2011).

In this learning I will be able to apply procedures such as ensuring the aptness of the patient are sitting position in avoiding aspiration and regurgitation among the patients. I will also be able to prepare medications, ensuring that I do not make silly mistakes such as giving the dose to the wrong patient therefore checking the identaband and asking the patient his/her name will be important, checking the bedside assessment to ensure that the medication has not been administered already and also ensuring that the patient swallows the medication. I will have the opportunity to ensure that I administer the correct medication, to the correct patient, the appropriate dosage, the right route and that the patient receives the medication at the suitable time and finally that the documentation is in order. These procedures may appear simple to execute but maximum care and experience is required because much is at stake in regard to the well being of the patient.

Oral medication will help me in identifying instances where the drug administered reacts negatively to a patient as a result of interacting with other drugs, irritation of the gastrointestinal tract and instances where the drug fast-passes through the body with no effect. The placement will help in making more sense to these complications.

Documentation helps in keeping a record of the work that has been done. It also helps in keeping tabs with what one is required to do. Nurses interact with the doctors through their documented materials. Details derived from the assessment and examination of the wound will have to be documented. The clinical placement will provide me with this opportunity. The oral medication process must also be well documented to show the patient’s recovery progress, the drugs and dosage that have been administered so that in case of ineffectiveness the documented work can be a reference point in determining other alternatives (Crisp et al., 2013). The placement will help me in taking notes as I routinely check on the patients. This forms a critical part of the nursing intervention.

In addition, the placement will be a suitable platform for practicing the ode ethics surrounding the nursing profession as well as the code of professional conduct. It will also be my basis for exercising my professional boundaries. Another important aspect that the placement will help me with is the decision making framework. The practical experience will help in identifying the scope of my decision making and consideration of my theoretical knowledge in decision making environments.

  • Let mentor know at the beginning of my shift what am looking forward to achieve that day.

  • Liase with my clinical facilitator to help in finding opportunities to attend to patients with wounds and those requiring oral medication.

  • Learn from observation of the interventions that are applied by other nurses.

  • Involving myself in practical assignments because no matter how much I observe it is not comparable to the practical knowledge.

  • Setting the day to day limits and involving my mentor or supervisor in facilitating this as a learning procedure.

  • Never procrastinating anything. Ensuring that I achieve my day to day goals as they fall due.

  • My mentor at the placement facility and will be helping me identify the opportunities to achieve my goals related to a wound dressing, oral medication and documentation.

  • The clinical supervisor will also be the focal point of my reference in regard to daily activities.

  • Constant review of the code of professional conduct and the ethical code will also help in ensuring that I act professionally without crossing the professional boundaries.

  • The placement facility policies such as the time for shifts, patient attendance policies and social policies will also contribute to the achievement of my goals.

The knowledge and experience I acquired in the placement was vast. I accomplished much more than I expected.

  • I achieved my objectives by learning from my mistakes. Reporting my mistakes and being accountable for them.

  • The feedback I got from my clinical supervisor and my mentor shaped achievement of my goals. Viewing their questioning and criticisms as a challenge or an opportunity to learn helped me a lot.

  • Observation and noting of how other professional nurses attended to their duties also contributed immensely to the achievement of my goals.

  • Attending of the debriefing sessions with fellow nurses and sharing my two sense in the debates got me so far. I was able to air issues that I was facing in my learning experience and had them addressed. I also learnt a lot in these sessions about the underpinning theory regarding wound dressing.
