Implementing models of clinical decision making in practice.

The aim of this assessment is to assist nurses to understand the health care issues in order to arrive at the correct clinical decision-making.

Making decisions on clinical matters is a very essential part of health care. In every decision-making regarding to the care of the patient, a medical practitioner plays a key role. Making a clinical decision will incorporate reasoning, judging, and drawing conclusions in a clinical manner. A sound decision leads to a practice that is effective and successive (Hardy & Smith, 2008).

This article explains student’s methods of improving critical thinking in making decisions on clinical matters. Each day the patients are developing complex problems, this challenge the nurses who are also required to make judgments that are complex on clinical matters. A very small percentage of nurses in their first job are able to apply critical thinking abilities in their profession. To improve the critical thinking, there is a need to put together the online learning with patient simulation to assist them in analyzing what they think in solving clinical matters. Students have different styles of learning and there is a need to develop exercises to link the online learning and simulation. Simulation instills the students’ confidence to deal with clinical matters.

To make analysis of skills in critical thinking, a complicated clinical situation is developed. Students are given an online case study to for discussion, and then there is a simulation process where students take different roles such as nurses, primary, family members and aides in a medical set up. Debriefing will assist the students in learning the critical problems that the patient is experiencing. The students are supposed to reflect on this assignment based on their own understanding.

The author concludes that this assignment benefits student as they became aware on the importance of assessment of a patient by considering the data provided and it also utilized their skills in critical thinking when analyzing the patient’s conditions. The students have the challenge of reflection based on their own views.

Critical thinking in novice nurses can be improved by involving nurses in process of decision making by assessment of patient with dietary complications. In this case the students under informed professionals will be required to participate in obtaining evidence that will be based on dietary options of treatment to their patients. This assignment will help them to clarify what is important to their patients on the choices of the diet.

This assignment will benefit the student as they become aware on the importance of assessment of a patient by considering the data provided. On the other hand, this will also utilize their skills in critical thinking when analyzing the patient’s conditions (Lichtenstein et al. 2006).

The article describes seclusion of mental health situations in hospitals in order to implement a framework on clinical decision-making. The objectives involved during this implementation are the identification of the cue that nurses apply in recognition of a patient who is at the risk of seclusion, to distinguish the considerations taken by the nurse to arrive to a decision of seclusion and coming up with a framework to direct nurses during their practice.

The nurses do not share common views but develop a new way of working together with an aim of realizing their potential.

These groups employ a series cycles for collecting and analyzing data, which will in turn assist them to come up with a decision-making framework.

During the project cycle, the project officer will have to explain the project in details then issue them with question for discussion and debating with their counterparts.

A clinical decision making framework will assist novice nurses to have a reflection of the decisions they have made then have a chance of developing the real features of clinicians who are experts. Nurses are able to make decisions based on documented information in addition to their basic understanding (Gillespie, Barbara & Peterson, 2009).

This article compares novice and expert nurses ways of assessing patients in order to make decision based on cue collection. The nurses must be able to realize any complication as early as possible and venture into the case when it is appropriate. For the nurses to understand the possibility of patient deterioration, nurses require accuracy in detecting the cues and the clusters of the cues. It appears that there is a total misunderstanding by nurses on the perception and the process of cues in making of decisions.

The case involves a group of novice nurses and expert nurses caring for patients with the same problems.

The author concludes that there is a wider collection of cues by expert nurses. The expert nurses went ahead in clustering the more that helps in making decision by identifying the patient’s status. Expert nurses took initiative by acting when collecting the cues of relevance and expected the problems, which help to indentify patient’s challenges. Novice nurses ended up in a hypothesis brought about single and minimal cues. They displayed lack of capability in discriminating the cues are salient and the ones that are not.

A guiding model assists novice nurses in analyzing the client’s cues for better decision making. In this model, the novices explain their reasoning so that they can fully understand what the client is experiencing. Instead of data collection to determine the truth through clarification of a hypothesis, they discuss and come to an agreement of the situation at hand. In this process, qualitative data is analyzed based on the reasoning of the therapist using the following conceptual frameworks; strategies on reasoning, combination of various strategies using the clients’ cues then discussing to obtain a common understanding. The aim of this study is determining how novice students make the decisions concerning diagnosis, about its treatment and the expected outcome. The study examines the critical thinking of the students to promote critical thinking capacity.

The study shows that the guided reflection is essential in determining the way students will make decisions especially during their first practice. There is a need for more assistance to mould a client centered treatment that will be outcome oriented (Wessel, Williams & Cole, 2006).

Conclusion

To be aware of what one does not know is the initial step required to change ones perception and behavior. Involving nurses in online discussions and the simulation of the patient gives them a chance of analyzing critical thinking in their own ways. This is an essential factor in clinical decision-making and results to improvement in learning and any easy evolution in the role of nursing. A guided reflection is essential in determining the way students will make decisions especially during their first practice.
